Rectoneovaginal fistulae (RnVFs) are rare but serious complications following vaginoplasty. Successful repair of recurrent RNVFs in transgender women can be achieved using buccal mucosa grafts.

- The initial repair attempt was unsuccessful.
Findings:
- The recurrent RNVF was successfully repaired using a buccal mucosa graft.
- Buccal mucosa grafting is a viable option for complex RNVF repair.
Implications:
- This case highlights the potential of buccal mucosa grafts in managing complex RNVFs.
- Multidisciplinary approaches may be necessary for optimal patient outcomes.
