Purpose of the Study:

  • To identify patient characteristics and care processes associated with reengagement in VA (Veterans Affairs) psychotherapy for PTSD.
  • To examine the relationship between various healthcare encounters and subsequent engagement in PTSD psychotherapy.

Main Methods:

  • A retrospective cohort study using national VA data of 24,492 veterans diagnosed with PTSD.
  • A nested case-control study compared veterans who reengaged in psychotherapy with those who did not.
  • Conditional logistic regression models were used to analyze sociodemographic, mental health, and service utilization factors.

Main Results:

  • 39.4% of veterans in the cohort reengaged in psychotherapy.
  • Encounters with primary care, primary care mental health, non-PTSD psychotherapy, other non-PTSD mental health care, other non-psychotherapy PTSD care, emergency room, and psychiatric hospitalization were all associated with increased odds of reengagement.
  • Specifically, other non-psychotherapy PTSD care showed a significant association (OR=3.31).

Conclusions:

  • A broad range of healthcare services received by veterans is linked to higher rates of reengagement in PTSD psychotherapy.
  • Healthcare providers across all settings should be aware of their role in supporting veterans' return to psychotherapy.
  • Integrated care approaches may enhance sustained engagement in mental health treatment for veterans with PTSD.
