Background:

Acute non-traumatic knee swelling presents a common diagnostic challenge in clinical practice.
Identifying the underlying cause is essential for effective treatment of knee effusions.

Purpose of the Study:

To provide an overview of potential diagnoses for non-traumatic knee swelling.
To suggest a structured algorithm for approaching non-traumatic knee arthritis.

Main Methods:

Thorough patient anamnesis to differentiate between inflammatory and non-inflammatory arthritis.
Joint aspiration and synovial fluid analysis (white cell count, microscopy, culture) when a degenerative cause is not apparent.
Evaluation of C-reactive protein levels.

Main Results:

Patient history is fundamental in differentiating arthritis types.
Synovial fluid analysis provides valuable diagnostic information for knee effusions.
Normal C-reactive protein and synovial fluid white cell count below 2,000/mm³ make septic arthritis unlikely.
