High-frequency jet ventilation (HFJV) during fentanyl anesthesia did not alter low-frequency heart rate variability. However, HFJV eliminated high-frequency variability and increased stress hormones, suggesting altered breathing patterns and sympathetic activity.
Area of Science:
- Anesthesiology
- Cardiovascular Physiology
- Respiratory Physiology
Background:
- Heart rate variability (HRV) reflects autonomic nervous system activity.
- High-frequency jet ventilation (HFJV) is an alternative mechanical ventilation strategy.
- Understanding HFJV's impact on cardiovascular regulation during anesthesia is crucial.
Purpose of the Study:
- To investigate the effects of HFJV on HRV during fentanyl anesthesia.
- To compare HRV parameters between HFJV and intermittent positive pressure ventilation (IPPV).
Main Methods:
- Power spectral density analysis of R-R interval time series.
- ECG and arterial pressure monitoring in nine patients under fentanyl anesthesia.
- Comparison of HRV (LF and HF power) between IPPV and HFJV.
Main Results:
- Low-frequency (LF) power of HRV remained unchanged between IPPV and HFJV.
- High-frequency (HF) power of HRV was detected during IPPV but not during HFJV.
- Plasma norepinephrine and epinephrine levels were significantly higher during HFJV compared to IPPV.
Conclusions:
- HFJV primarily affects the respiratory frequency component of HRV during fentanyl anesthesia.
- Altered breathing patterns and increased sympathetic neural outflow may contribute to observed HRV changes.
- HFJV's influence on cardiovascular autonomic regulation warrants further investigation.

Cardiac Output I:Effect of Heart Rate on Cardiac Output
Cardiac output (CO) refers to the total amount of blood ejected by one of the ventricles in liters per minute (L/min). In a resting adult, CO ranges from 5 to 6 L/min, adjusting according to the body's metabolic requirements.
Effect of Heart Rate on Cardiac Output
Cardiac output adapts to metabolic demands during stress, physical activity, or illness. The autonomic nervous system regulates heart rate via the sinoatrial node. The parasympathetic nervous system decreases heart...
Factors Affecting Pulmonary Ventilation
Alveolar Surface Tension
The alveolar fluid lines the luminal surface of the alveoli and exerts a force called surface tension. This force is caused by the polar water molecules in the liquid being more strongly attracted to each...
Assessment of Ventilation I: Respiratory Rate
A Ventilation assessment is critical for monitoring a patient's health status. Respiration, one of the most accessible vital signs, provides insights into the function of numerous body systems and can indicate serious health issues, such as brainstem injuries from head trauma.
Critical Guidelines for Assessing Ventilation:
Regulation of Heart Rates
The SNS increases heart rate through the release of norepinephrine and epinephrine, which act on beta-1 adrenergic receptors in the heart. This action increases the rate of depolarization in the sinoatrial (SA) node, the heart's...
