Area of Science:

  • Orthopedic surgery
  • Radiology
  • Medical imaging analysis

Background:

  • The Schatzker classification is a standard method for categorizing tibial plateau fractures.
  • Evaluating fracture patterns requires accurate and reproducible classification systems.
  • The increasing use of mobile technology in medicine prompts investigation into its diagnostic utility.

Purpose of the Study:

  • To assess the intra-observer reliability of the Schatzker classification system for tibial plateau fractures when images are viewed on smartphones.
  • To compare the reproducibility of Schatzker classification using smartphone-based image viewing versus traditional assessment methods.

Main Methods:

  • 37 patients with tibial plateau fractures underwent imaging (radiographs and CT scans).
  • Two orthopedic surgeons classified fractures twice using isolated radiographs and then combined X-ray/CT images, assessed via smartphone and in-person.
  • The Kappa coefficient (k) was used for statistical analysis of intra-observer agreement.

Main Results:

  • High intra-observer agreement was observed when comparing smartphone-based evaluation with traditional assessment methods.
  • Statistical analysis confirmed the reliability of using smartphones for Schatzker classification.
  • No significant decrease in classification accuracy was noted with smartphone use.

Conclusions:

  • Smartphone applications are a reliable tool for the intra-observer classification of tibial plateau fractures using the Schatzker system.
  • The use of smartphones does not compromise the reproducibility of this fracture classification.
  • This finding supports the potential integration of mobile technology in orthopedic fracture assessment.
