Endometriosis in the extraperitoneal round ligament (EERL) is rare, often right-sided, and linked to inguinal hernias and reduced fertility. Consider co-occurring intraperitoneal endometriosis in sterile patients with EERL.

Background:

  • Endometriosis is a condition where tissue similar to the uterine lining grows outside the uterus.
  • Extraperitoneal endometriosis, particularly in the round ligament, is an uncommon presentation.

Observation:

  • A recent case of endometriosis in the extraperitoneal round ligament (EERL) was identified.
  • A literature review identified 30 previously reported cases of EERL.
  • The majority (90%) of EERL cases were located on the right side.
  • EERL was frequently associated with inguinal hernias.

Findings:

  • Fertility appeared to be reduced in patients with EERL.
  • Four cases involving laparotomy revealed concurrent intraperitoneal endometriosis.
  • This suggests a potential link between EERL and deeper pelvic endometriosis.

Implications:

  • The presence of EERL, especially in cases of infertility, warrants consideration for coexisting intraperitoneal endometriosis.
  • Accurate diagnosis and surgical planning are crucial for managing EERL and associated conditions.
  • Further research may elucidate the pathogenesis and clinical significance of EERL.
