Summary

Postoperative tracheal compression from hematoma is rare after cardiovascular surgery. Prompt diagnosis and surgical removal of hematoma following aortic surgery can resolve severe respiratory distress.

  • Computed tomography revealed tracheal bifurcation obstruction due to a hematoma associated with Surgicel packing.

Findings:

  • Surgicel packing, used for hemostasis, contributed to a significant postoperative hematoma.
  • Reexploration and removal of the hematoma led to rapid improvement in respiratory status.

Implications:

  • Tracheal compression by hematoma around surgical packing should be considered in unexplained postoperative respiratory failure.
  • This case highlights the importance of vigilant monitoring for rare complications in cardiovascular surgery patients.
