The alternative DSM-5 model for personality disorders (PDs) shows reduced diagnostic overlaps in China. This simplified approach is recommended over the current DSM-5 PD system for better clinical practice.
Area of Science:
- Psychiatry
- Clinical Psychology
- Mental Health Research
Background:
- The Diagnostic and Statistical Manual-5 (DSM-5) outlines personality disorders (PDs).
- Overlapping PD diagnoses present challenges in clinical settings.
- Evaluating alternative diagnostic models is crucial for improving accuracy.
Purpose of the Study:
- To investigate the degree of overlap among DSM-5 Personality Disorders in a Chinese high-risk clinical population.
- To explore a transitional model for implementing DSM-5 PDs.
- To assess the suitability of the alternative DSM-5 model for personality disorders in China.
Main Methods:
- A sample of 982 outpatients with at least one diagnosed PD was recruited from the Shanghai Mental Health Center.
- Diagnostic assessments included a personality diagnostic questionnaire and a structured clinical interview.
- Cluster and principal component analyses were employed to explore diagnostic models.
Main Results:
- 22.3% of patients met criteria for at least one of six PDs under the alternative DSM-5 model.
- 20.3% of patients with PD met criteria for at least two PDs.
- A transitional model with 7 PD categories showed a 24.1% overlap rate, suggesting better appropriateness for China.
Conclusions:
- The alternative DSM-5 model for personality disorders simplifies diagnoses and reduces overlaps in Chinese psychiatric practice.
- This model is preferable to the current DSM-5 PD diagnostic system.
- Implementing simplified PD categories enhances diagnostic clarity and clinical utility.
