Summary

Mandibular setback surgery reduces lower facial width at the mandibular angle. This decrease is evident immediately after surgery and continues to progress during the postoperative period, as shown by 3-D CT analysis.

Background:

  • Mandibular prognathism correction often involves mandibular setback surgery.
  • Understanding the dimensional changes of the proximal mandibular segment is crucial for surgical planning and outcome assessment.

Purpose of the Study:

  • To evaluate horizontal changes in the proximal mandibular segment after mandibular setback surgery.
  • To assess changes in intercondylar width, intergonial width, and frontal-ramal inclination using 3-D CT.

Main Methods:

  • A retrospective study of 66 patients undergoing bilateral sagittal split ramus osteotomy for mandibular prognathism.
  • 3-D CT scans were acquired preoperatively (T1), immediately postoperatively (T2), and 6 months postoperatively (T3).
  • Horizontal dimensions and angulation of the proximal mandibular segment were analyzed.

Main Results:

  • The intergonial width significantly decreased at 6 months postoperatively compared to preoperatively (P < .001).
  • Frontal-ramal inclination showed changes on both sides, with the right side decreasing from T2 to T3 and the left side showing a slight decrease from T2 to T3.
  • Intercondylar width demonstrated minimal changes throughout the study period.

Conclusions:

  • Lower facial width at the mandibular angle region decreases immediately after mandibular setback surgery.
  • Further reduction in lower facial width at the mandibular angle is observed during the postoperative period.
