Background:

Intracranial prepontine cysts, including epidermoid cysts, are rare and can cause symptoms requiring surgical intervention.
Spontaneous resolution of these cysts is exceptionally uncommon in medical literature.

Observation:

A young male presented with headache and fever, diagnosed via MRI with a prepontine epidermoid cyst.
The patient received symptomatic treatment, empirical antibiotics, and dexamethasone during admission.

Findings:

The patient experienced symptomatic improvement within 48 hours and was discharged.
Follow-up imaging at 6 months and 1 year revealed a significant reduction in the cyst's size.

Implications:

Conservative management and symptomatic treatment may lead to spontaneous resolution of symptomatic prepontine epidermoid cysts.
Close radiological and clinical monitoring is advised for asymptomatic prepontine cysts.
