Background:

  • Radiographs are vital for assessing crown fit, particularly at interproximal margins where tactile and visual methods are limited.
  • Limited research exists on the effectiveness of digital radiography for evaluating ceramic crown marginal openings.

Purpose of the Study:

  • To determine if digital radiographs can accurately identify marginal adaptation in ceramic and metal-ceramic crowns.
  • To assess the diagnostic utility of digital radiographs for evaluating crown fit in vitro.

Main Methods:

  • Fabricated lithium disilicate, fluorapatite, and metal-ceramic crowns for a maxillary premolar.
  • Created controlled marginal discrepancies (0-200 μm) and captured digital radiographs at different angles.
  • 21 dentists evaluated radiographs for margin acceptability (≤80 μm considered unacceptable).

Main Results:

  • Metal-ceramic crowns were frequently misclassified as acceptable (78.6% false positives) for discrepancies >80 μm.
  • Ceramic crowns showed higher rates of false negatives, being incorrectly deemed unacceptable (lithium disilicate 66.1%, fluorapatite 45.8%).

Conclusions:

  • Digital radiography alone is insufficient for accurately evaluating the marginal fit of complete coverage crowns.
  • Metal-ceramic crowns' fit may be overestimated, while ceramic crowns' fit may be underestimated using digital radiographs.
  • Clinical examination is essential for accurate marginal adaptation assessment.
