Summary

Achieving the Lupus Low Disease Activity State (LLDAS) for at least two years significantly reduces damage accrual in systemic lupus erythematosus (SLE) patients. Most patients in LLDAS also met remission criteria, contributing to this protective effect.

Background:

  • Systemic lupus erythematosus (SLE) is a chronic autoimmune disease characterized by variable disease activity and potential for irreversible organ damage.
  • Defining and achieving low disease activity states is crucial for improving long-term outcomes in SLE patients.

Purpose of the Study:

  • To assess the prevalence and duration of the Lupus Low Disease Activity State (LLDAS) in a cohort of SLE patients.
  • To evaluate the impact of achieving LLDAS on the accrual of long-term damage in SLE.

Main Methods:

  • A 7-year follow-up of 293 Caucasian SLE patients was conducted.
  • Disease activity was measured using SLEDAI-2K and physician global assessment (PGA). Damage was assessed using the Systemic Lupus International Collaborating Clinics/American College of Rheumatology Damage Index (SDI).
  • LLDAS was defined by specific criteria including low SLEDAI-2K, minimal PGA, and low-dose prednisone. Remission criteria were also evaluated.

Main Results:

  • LLDAS was achieved by a significant proportion of patients, with 37.2% maintaining it for ≥5 consecutive years.
  • Patients achieving LLDAS for at least two consecutive years showed significantly less damage accrual (p=0.001) and were less likely to experience an increase in SDI (OR 0.160, p<0.001).
  • On average, 84% of patients in LLDAS also met criteria for remission.

Conclusions:

  • Sustained Lupus Low Disease Activity State (LLDAS) is associated with reduced damage progression in SLE.
  • The high rate of remission among patients in LLDAS likely contributes to its protective effect against long-term damage accrual.
  • These findings support LLDAS as a valuable therapeutic target in managing SLE.
