Late-onset self-healing Langerhans cell histiocytosis (LCH) is a rare condition presenting with skin lesions that resolve spontaneously. This case highlights the importance of monitoring patients for systemic involvement to confirm the diagnosis retrospectively.

Area of Science:

  • Dermatology
  • Pediatrics
  • Pathology

Background:

  • Langerhans cell histiocytosis (LCH) is a rare clonal proliferation of Langerhans cells.
  • Self-healing LCH is a rare, self-limited variant typically presenting in neonates.
  • Late-onset LCH is exceptionally rare, with few reported cases.

Observation:

  • A 4.5-month-old infant presented with an erythematopurpuric eruption.
  • Skin biopsy confirmed isolated cutaneous Langerhans cell histiocytosis.
  • Lesions spontaneously regressed within months.

Findings:

  • The patient was retrospectively diagnosed with late-onset self-healing LCH.
  • Diagnosis was confirmed after four years of follow-up with no skin or systemic involvement.
  • Late-onset LCH can present with purpuric lesions beyond the neonatal period.

Implications:

  • Late-onset self-healing LCH requires retrospective diagnosis after prolonged observation.
  • Purpuric lesions in infants may indicate LCH and warrant biopsy.
  • Regular monitoring for systemic involvement is crucial for patients diagnosed with cutaneous LCH.
