Background:

  • Blood drug analysis is key for determining cause of death in postmortem cases.

  • Limited information exists on comparing drug detection in postmortem blood versus hair.

Purpose of the Study:

  • To compare the detection of psychoactive medicinal drugs in postmortem blood and hair samples.
  • To evaluate the utility of hair analysis for identifying drug use not evident in blood.

Main Methods:

  • Collected blood and hair samples from 55 forensic autopsies.
  • Analyzed samples for 39 medicinal drugs (benzodiazepines, antidepressants, antipsychotics).
  • Utilized a validated liquid chromatography-tandem mass spectrometry method.

Main Results:

  • Hair analysis revealed drug use in 85% of cases where blood tests were negative.
  • Benzodiazepines (64%) and antidepressants (35%) were most frequently detected in hair but not blood.
  • In 4% of cases, benzodiazepines were in blood but not hair, suggesting potential reduced tolerance.

Conclusions:

  • Hair analysis significantly enhances the detection of prior drug use in postmortem investigations.
  • This method can provide vital information for diagnosing unknown psychiatric conditions.
  • Discrepancies in detection may indicate variations in drug tolerance.
