Summary

Asymptomatic creatine kinase elevation can occur with oral neuroleptic treatment. A patient tolerated a different neuroleptic, suggesting individualized responses to these medications.

Background:

  • Neuroleptic medications are commonly prescribed for psychiatric conditions.
  • Elevated creatine kinase (CK) levels can indicate muscle damage or stress.
  • Asymptomatic CK elevation is an uncommon but documented side effect of certain drugs.

Observation:

  • Two patients presented with asymptomatic elevations in serum creatine kinase (CK) levels during oral neuroleptic therapy.
  • No overt symptoms of muscle injury or neurological dysfunction were reported by either patient.
  • One patient underwent a dechallenge and rechallenge protocol.

Findings:

  • The patient who underwent rechallenge tolerated a different oral neuroleptic agent without recurrence of elevated CK levels.
  • This suggests that the specific neuroleptic agent, rather than the class, may be implicated in CK elevation.
  • The underlying mechanisms for neuroleptic-induced asymptomatic CK elevation remain unclear.

Implications:

  • Clinicians should be aware of the potential for asymptomatic CK elevation with oral neuroleptics.
  • Monitoring CK levels may be warranted in patients on specific neuroleptic agents.
  • Further research is needed to elucidate the pathophysiology and clinical significance of this finding.
