Background:

Traumatic penile amputation is a rare pediatric surgical emergency.
Established penile replantation techniques exist, but failure causes are poorly documented.
This report details a case of penile replantation in a 9-year-old boy.

Observation:

Microsurgical replantation using papaverine was performed for penile amputation.
Post-operative complications included glans discoloration within 48 hours and necrosis by day 4.
Significant loss of the re-implanted penis and overlying skin occurred by day 12.

Findings:

Papaverine aided microsurgical penile replantation in a pediatric case.
Delayed necrosis and partial loss of the replanted penis were observed.
A novel technique using a pediatric cannula for papaverine instillation was employed.

Implications:

Penile replantation in pediatric patients presents significant surgical challenges.
The described technique, despite its failure in this case, may offer value in resource-limited settings.
Further research into optimizing penile replantation outcomes in children is warranted.
