Background:

Pancreatoduodenectomy is a complex procedure with high complication rates (30-61%).
Minimizing complications and mortality necessitates high-quality, standardized surgical techniques.
Surgeon experience is critical for successful pancreatoduodenectomy outcomes.

Purpose of the Study:

To investigate current practice patterns for pancreatoduodenectomy in Brazil.
To identify variations in surgical techniques and perioperative management.
To compare Brazilian practices with established literature recommendations.

Main Methods:

A survey questionnaire was developed to assess surgical practice, training, and experience.
The survey targeted surgeons with declared interest in pancreatic surgery.
Data on surgical approach, reconstruction methods, and postoperative care were collected.

Main Results:

52 Brazilian surgeons responded to the survey (86.7% return rate).
Laparoscopic pancreatoduodenectomy was infrequently used (<4%).
Significant heterogeneity was found in reconstruction techniques (e.g., pancreaticojejunostomy 94.2%, Braun enteroenterostomy 11.5%) and perioperative management (e.g., somatostatin use 11.5%).

Conclusions:

Brazilian pancreatoduodenectomy practices exhibit considerable heterogeneity.
Some observed practices deviate from current evidence-based guidelines.
Standardization of techniques may improve outcomes for this challenging surgery.
