Multiple endocrine neoplasia type I (MEN I) patients with hyperparathyroidism may exhibit abnormal gastrin secretion. However, G-cell function remains normal in gene carriers without gastrinoma, indicating gastrinoma as the sole cause of abnormal gastrin in MEN I families.
