Bronchial artery to pulmonary circulation shunt (BPS) is a rare condition in children presenting with recurrent cough and anemia. Transcatheter embolization effectively treated these symptoms, offering a new therapeutic avenue.

Background:
- Recurrent cough and anemia in children can mimic idiopathic pulmonary hemosiderosis.
- Idiopathic pulmonary hemosiderosis is often treated with corticosteroids.
Observation:
- Four young girls presented with recurrent cough, anemia, and shortness of breath.
- Bronchoalveolar lavage fluid tested positive for hemosiderin; imaging showed ground-glass opacity or interstitial changes.
- Patients unresponsive to idiopathic pulmonary hemosiderosis treatment were investigated further.
Findings:
- Selective bronchial arteriography revealed bronchial artery to pulmonary circulation shunt (BPS) in all four girls.
- Transcatheter embolization was performed to correct the abnormal shunts.
- A six-month follow-up showed no recurrence of cough and anemia.
Implications:
- Bronchial artery to pulmonary circulation shunt (BPS) should be considered in pediatric cases of unexplained recurrent cough and anemia, especially those refractory to corticosteroid therapy.
- Transcatheter embolization is a successful treatment for BPS in children.
- Early diagnosis and intervention for BPS can prevent persistent respiratory and hematological symptoms.
