Background:

  • Erectile dysfunction (ED) is a common condition often linked to underlying cardiovascular issues.
  • Endocan, a proteoglycan, has emerged as a potential biomarker in various vascular diseases.

Purpose of the Study:

  • To measure serum endocan levels in patients with erectile dysfunction (ED).
  • To investigate the association between serum endocan levels and the severity of vasculogenic ED.

Main Methods:

  • Prospective analysis of 86 consecutive male patients with ED.
  • Patients categorized into severe ED (IIEF-5 score < 7) and mild/moderate ED (IIEF-5 score > 7).
  • Serum endocan levels, peak systolic velocity, and other clinical parameters were assessed.

Main Results:

  • A strong negative correlation was observed between serum endocan levels and peak systolic velocity in severe ED patients (p < .001, r = -.665).
  • Univariate analysis identified tobacco consumption, low vitamin D, higher serum endocan, lower peak systolic velocity, hypertension, dyslipidemia, metabolic syndrome, and cardiovascular events as risk factors for severe ED.
  • Multivariate analysis confirmed age, hypertension, metabolic syndrome, cardiovascular events, and higher serum endocan levels as independent predictors of severe ED.

Conclusions:

  • Circulating endocan levels are significantly associated with the severity of erectile dysfunction.
  • Endocan may serve as a novel biomarker in clinical practice, correlating with cardiovascular risks and ED severity.
