    Background:

    • Postpartum headaches are a frequent concern for women after childbirth.
    • Understanding the incidence and causes of these headaches is crucial for maternal health.

    Purpose of the Study:

    • To determine the incidence and causes of headaches in the first month postpartum.
    • To differentiate between primary and symptomatic headaches.
    • To identify predictors of different headache types.

    Main Methods:

    • Prospective study of 900 women within 3 days of delivery.
    • Clinical examination, transcranial color-coded sonography (TCCS), MRI, and angiography for suspected cases.
    • Telephone interviews at 1 month postpartum.
    • Statistical analysis including logistic regression.

    Main Results:

    • 26.8% of women reported headaches in the first postpartum month.
    • Thunderclap headache occurred in 3.8% of subjects, mostly benign.
    • Gradual onset headache in 23% of subjects; pre-eclampsia and postural headache post-dural anesthesia were identified.
    • Migraine history and parity were predictors of gradual onset, pulsating headaches.

    Conclusions:

    • Headache incidence increases in the first postpartum month.
    • Predictors vary based on headache presentation (gradual vs. thunderclap).
    • The vast majority of postpartum headaches are primary and not indicative of serious pathology.
