    Background:

    • Post-hypoxic intention myoclonus is a neurological disorder potentially linked to deficient central serotonergic tone.
    • Serotonin, a key neurotransmitter, plays a crucial role in regulating motor control and mood.

    Observation:

    • Patients with post-hypoxic intention myoclonus often exhibit reduced levels of 5-hydroxyindoleacetic acid (5-HIAA) in cerebrospinal fluid.
    • Administration of serotonin precursors, tryptophan and 5-hydroxytryptophan, was observed to increase 5-HIAA levels.

    Findings:

    • Tryptophan and 5-hydroxytryptophan administration effectively suppressed myoclonus in patients with post-hypoxic intention myoclonus.
    • However, serotonin precursor administration was not universally effective and could exacerbate certain myoclonic syndromes, such as those associated with lipid storage diseases.

    Implications:

    • These findings suggest a potential therapeutic role for serotonin precursor supplementation in specific myoclonic syndromes.
    • Further research is needed to elucidate the precise mechanisms and identify patient subgroups who would benefit most, while considering potential adverse effects.
