Distinct pathogenesis in nonsystemic vasculitic neuropathy and microscopic polyangiitis

Nonsystemic vasculitic neuropathy (NSVN) and microscopic polyangiitis (MPA) involve different vasculitis mechanisms. Complement drives NSVN pathogenesis, while neutrophil attachment to endothelial cells initiates MPA vasculitis, irrespective of antineutrophil cytoplasmic antibodies (ANCAs).

- NSVN and MPA exhibit distinct vasculitic mechanisms.
- In MPA, neutrophil attachment to vascular endothelium appears to be an initial pathogenic event, independent of ANCA status.
- Complement activation plays a significant role in the pathogenesis of vasculitis in NSVN.
