Bone remodeling is a continuous and balanced process of bone resorption by osteoclasts and bone formation by osteoblasts. In adults, it helps maintain bone mass and calcium homeostasis. While mechanical stress can stimulate turnover as part of the normal maintenance and reparative process, several hormones also regulate bone remodeling.

Background:

  • Conventional reconstruction methods are limited by poor vascularization after tumor treatments.
  • Regenerative medicine offers alternatives but faces challenges in irradiated tissues.

Purpose of the Study:

  • To investigate bone formation patterns in axially vascularized tissue engineering constructs (AVTECs) post-irradiation.
  • To mimic clinical scenarios following head and neck cancer treatment.

Main Methods:

  • AVTECs were created using Nanobone granules, bone marrow, and bone morphogenic protein-2.
  • An arteriovenous loop was implanted for axial vascularization.
  • Constructs were irradiated (15 Gy) and analyzed for bone formation, vascularization, and cellular activity.

Main Results:

  • Homogenous vascularization and bone formation were observed throughout the constructs.
  • Significantly greater bone formation occurred in the central regions compared to peripheral regions.
  • Vascularity, proliferation, and apoptosis were similar between central and peripheral areas.

Conclusions:

  • This study demonstrates bone formation patterns in AVTECs after clinically relevant irradiation doses.
  • Axial vascularization appears to enhance osteoinductive properties in the central regions of AVTECs post-irradiation.
