Background:

  • The California Right Care Initiative established the University of Best Practices (UBP) in 2011.
  • UBP is a countywide physician organization learning collaborative focused on managing cardiovascular and cerebrovascular risk factors.

Purpose of the Study:

  • To evaluate the association between the UBP initiative and hospitalizations for heart attacks and strokes.

Main Methods:

  • A quasi-experimental design compared hospitalization rates before (2007-2010) and after (2011-2014) UBP implementation.
  • Difference-in-differences (DID) logistic regression models analyzed hospitalization data from 2007-2014.
  • The study included over 370,000 heart attack and 640,000 stroke hospitalizations.

Main Results:

  • A significant decrease in heart attack hospitalizations was observed in San Diego County post-UBP (OR, 0.84; P <.001).
  • This reduction represented 2735 fewer hospitalizations, equating to $61 million in savings.
  • No significant association was found between UBP and stroke hospitalizations.

Conclusions:

  • Physician organization learning collaboratives can reduce heart attack hospitalizations.
  • Implementing such collaboratives may lead to decreased healthcare costs.
  • Further research is needed to explore stroke prevention strategies within these models.
