  • A patient undergoing chronic hemodialysis developed C. difficile colitis.
  • This occurred during monotherapy with intravenous vancomycin for presumed cervical osteomyelitis.

Findings:

  • This is the first reported case of C. difficile colitis linked to intravenous vancomycin monotherapy.
  • Discontinuation of vancomycin and oral metronidazole led to patient recovery.

Implications:

  • Prolonged elevated vancomycin levels in renal failure patients may increase susceptibility to C. difficile colitis.
