Extrapancreatic solid pseudopapillary tumors (SPT) are rare malignant pancreatic neoplasms. This case highlights aggressive extrapancreatic SPT with peritoneal dissemination and liver metastases, emphasizing the need for vigilance in diagnosis.

  • Solid pseudopapillary tumor (SPT) is typically a low-grade malignant pancreatic neoplasm.
  • Extrapancreatic occurrence of SPT is exceptionally rare, posing diagnostic challenges.

Observation:

  • A 61-year-old woman presented with abdominal pain and diffuse tenderness.
  • Imaging revealed extrapancreatic SPT with extensive peritoneal dissemination and hepatic metastases.

Findings:

  • This case represents the first report of extrapancreatic SPT exhibiting aggressive behavior and a poor prognosis.
  • Imaging findings mimicked classical pancreatic SPTs, but malignant transformation was suspected due to capsular discontinuity and invasion.
