Summary

Juxtaarticular intraspinal synovial cysts, rare spinal lesions linked to facet arthropathy, can mimic other conditions and cause radicular pain. Magnetic resonance (MR) imaging effectively visualizes these cysts and potential hemorrhage.

Area of Science:

  • Spine imaging
  • Radiology
  • Neurosurgery

Background:

  • Juxtaarticular intraspinal synovial cysts are uncommon spinal lesions.
  • They are frequently associated with facet arthropathy.
  • These cysts can present with radicular symptoms, mimicking other spinal pathologies.

Purpose of the Study:

  • To describe the magnetic resonance (MR) imaging characteristics of juxtaarticular intraspinal synovial cysts.
  • To evaluate the utility of MR imaging in diagnosing these lesions and identifying associated findings like hemorrhage.

Main Methods:

  • Retrospective review of six cases with lumbar intraspinal synovial cysts.
  • Detailed analysis of MR imaging features, including signal intensity and lesion location.
  • Correlation with clinical presentation and other imaging modalities where available.

Main Results:

  • Six juxtaarticular intraspinal synovial cysts in the lumbar spine were identified on MR imaging.
  • MR imaging demonstrated variable signal intensity patterns within the cysts.
  • Hemorrhage within a cyst was visualized on MR imaging, potentially correlating with symptom exacerbation.
  • Air-filled cysts were noted as potentially challenging to differentiate from facet arthropathy.

Conclusions:

  • MR imaging is a valuable tool for diagnosing juxtaarticular intraspinal synovial cysts.
  • MR imaging can characterize cyst contents, including hemorrhage, and aid in differentiating them from other spinal pathologies.
  • Understanding MR imaging findings is crucial for accurate diagnosis and management of these rare lesions.
