Atlanto-occipital rotatory dislocation (AORD) is a rare cervical spine injury. This study details a case and literature review, finding AORD unique with better outcomes than typical dislocations.

  • Atlanto-occipital rotatory dislocation (AORD) is infrequently documented, leading to limited understanding of its clinical and radiological features.
  • A comprehensive literature review was conducted to collate existing AORD cases.

Observation:

  • A 67-year-old male presented with neck pain following cervical trauma, maintaining neurological integrity.
  • CT imaging revealed rotatory displacement of atlanto-occipital joints with an increased condylar-C1 interval.
  • MRI demonstrated associated injuries to the stabilizing ligaments of the atlanto-occipital region.

Findings:

  • The patient underwent successful occiput-to-C1 craniocervical fixation.
  • The literature review identified 9 prior AORD cases.
