Summary

Transient primary hypothyroidism is increasingly reported, but thyroid hormone withdrawal is rarely beneficial. Most cases of primary hypothyroidism, excluding specific causes or postpartum onset, are permanent, making withdrawal unnecessary and not cost-effective.

Area of Science:

  • Endocrinology
  • Thyroidology

Background:

  • Transient primary hypothyroidism is increasingly diagnosed.
  • Physicians face uncertainty regarding the utility and cost-effectiveness of thyroid hormone withdrawal to assess transient hypothyroidism.

Purpose of the Study:

  • To evaluate the frequency of thyroid function recovery in patients with primary hypothyroidism.
  • To determine the indications and cost-effectiveness of thyroid hormone withdrawal for assessing transient hypothyroidism.

Main Methods:

  • Thyroid hormone therapy was withdrawn from 63 patients with proven primary hypothyroidism at 6 months, 1 year, and 3 years.
  • Patients were categorized based on the cause of hypothyroidism, including specific exclusions like drug therapy, surgery, or thyroiditis.

Main Results:

Related Experiment Videos

  • Of 49 patients with primary hypothyroidism (PH) not linked to specific causes, only two recovered thyroid function.
  • In 14 patients with postpartum-onset hypothyroidism, nine recovered thyroid function.
  • Thyroid hormone withdrawal was deemed unnecessary and not cost-effective for assessing recovery in most PH cases.

Conclusions:

  • Primary hypothyroidism, when unrelated to specific causes or postpartum states, is typically permanent.
  • Withdrawing thyroid hormone therapy to assess recovery is generally not indicated nor cost-effective for patients with primary hypothyroidism.
