Abstract:
The Prado Cardio programme is designed to support patients' return home and helps to identify the early warning signs of heart decompensation, thereby avoiding unscheduled hospitalisation. It also gives private practice nurses the chance to develop new expertise. The follow-up is personalised and established jointly with the patient and the multi-professional team.
