Background:

Neonatal appendicitis is a rare but life-threatening condition.
Prompt diagnosis and surgical management are critical for survival.

Observation:

A case of neonatal appendicitis in southern Brazil presented with food intolerance, hypoactivity, and septicemia due to intestinal perforation.
The condition was not diagnosed preoperatively, a common challenge in neonatal cases.

Findings:

Management of neonatal appendicitis is exclusively surgical.
Clinical presentation can mimic other neonatal abdominal emergencies.

Implications:

Close clinical surveillance for peritoneal irritation in neonates is essential.
Multidisciplinary collaboration between nursing and medical teams can facilitate early surgical intervention.
Timely intervention can prevent severe complications like septicemia and mortality.
