  • Understanding the utility of these embedded PVTs is vital for accurate diagnosis and treatment planning in clinical populations.

Purpose of the Study:

  • To validate and replicate the efficacy of embedded performance validity tests (PVTs) within the HVLT-R and BVMT-R.
  • To assess the ability of these embedded PVTs to detect noncredible responding in a clinical sample of veterans.
  • To determine the sensitivity and specificity of the HVLT-R and BVMT-R embedded PVTs.

Main Methods:

  • Eighty clinically referred veterans undergoing neuropsychological evaluation were included in the study.
  • Participants were categorized into valid (n=60) and noncredible (n=20) performance groups based on established PVTs.
  • The HVLT-R Recognition Discrimination (RD) and BVMT-R Percent Retained were analyzed for their ability to identify noncredible performance.

Main Results:

  • The HVLT-R RD score of ≤5 demonstrated 67% sensitivity and 80% specificity for detecting noncredible performance.
  • After excluding valid participants with an amnestic profile, specificity increased to 92%, replicating prior findings.
  • Replication efforts for the BVMT-R Percent Retained were unsuccessful, but significant findings for the HVLT-R RD were confirmed.

Conclusions:

  • The embedded PVT within the HVLT-R reliably identifies invalid performance in a diverse clinical veteran sample.
  • The findings corroborate the utility of the HVLT-R embedded PVT in detecting noncredible effort, even in individuals with cognitive impairment.
  • Further research may be needed to refine or identify alternative embedded PVTs for the BVMT-R.
