Summary

Early gastric cancer survival rates are lower in the US compared to Japan, particularly for submucosal lesions. Factors like patient age and associated non-gastric malignancies may influence outcomes.

Area of Science:

  • Gastroenterology
  • Oncology
  • Epidemiology

Background:

  • Early gastric cancer (EGC) diagnosis is crucial for improved patient outcomes.
  • Understanding demographic and clinical differences in EGC between regions is important for targeted treatment strategies.

Purpose of the Study:

  • To analyze the characteristics and survival rates of early gastric cancer patients in the United States.
  • To compare US-based EGC data with findings from a large Japanese study.

Main Methods:

  • Retrospective analysis of 36 patients with early gastric cancer.
  • Kaplan-Meier survival estimates were calculated based on lesion depth (mucosal vs. submucosal).
  • Comparison of patient demographics, tumor characteristics, and survival data with a Japanese cohort.

Main Results:

  • Of 36 patients, 15 had mucosal and 21 had submucosal invasion; 4 had lymph node involvement.
  • Five-year survival rates were 92% for mucosal and 65% for submucosal lesions (overall 73%).
  • US EGC patients were older and tumors developed more distally compared to Japanese patients; survival rates were less favorable.

Conclusions:

  • Submucosal invasion significantly impacts survival in early gastric cancer.
  • Differences in patient age and a higher incidence of associated non-gastric malignancies may contribute to less favorable survival outcomes in the US.
  • Further research is needed to elucidate the factors affecting early gastric cancer prognosis in different populations.
