Background:

  • Sentinel lymph node biopsy (SLNB) is a debated procedure for large tumors.
  • Understanding its false negative rate (FNR) in this population is crucial.

Purpose of the Study:

  • To evaluate the FNR of SLNB in patients with large breast tumors.
  • To identify predictive factors for false negatives (FN) in SLNB for large tumors.

Main Methods:

  • Multicentric cohort study of N0 breast cancer patients undergoing SLNB.
  • Analysis of FNR and predictive factors for FN.
  • Comparison of outcomes based on tumor size: ≥30 mm, >20 to <30 mm, and ≤20 mm.

Main Results:

  • FNR for tumors ≥30 mm was 3.05%, for >20 to <30 mm was 3.5%, and for ≤20 mm was 1.8%.
  • Tumor size >20 to <30 mm (OR: 2.07) and fewer than 3 lymph nodes harvested (OR: 2.0) were significant predictors of FN.
  • Tumor size ≥30 mm was not a significant predictor of FN (OR: 1.83).

Conclusions:

  • The FNR of SLNB is not significantly higher for large breast tumors compared to smaller ones.
  • SLNB is validated for tumors up to 50 mm, supporting its use in larger breast cancers.
