Background:

  • Basal cell carcinoma (BCC) is the most common human cancer.
  • Granular cell BCC is a rare histopathological variant with limited literature.

Observation:

  • A 47-year-old man presented with an ulceration on his right upper lip.
  • The lesion was surgically removed and examined histopathologically.

Findings:

  • The tumor consisted exclusively of granular cells with eosinophilic inclusions.
  • High mitotic activity (8-15/hpf) and a Ki-67 index of ~5% were noted.
  • Immunohistochemistry revealed positivity for Ber-EP4, pan-cytokeratin, AE1/AE3, CK5/6, p63, lysozyme, CD68, and Bcl-2.

Implications:

  • This case expands the understanding of granular cell BCC morphology and immunophenotype.
  • Further research may clarify the origin and behavior of this rare BCC variant.
