There are hollow fluid-filled cavities known as ventricles deep inside the human brain. There are two lateral ventricles, one in each cerebral hemisphere, and each has three different projections — the anterior, inferior, and posterior horns visible from the lateral side. A thin membrane called the septum pellucidum separates the two lateral ventricles. The slender third ventricle in the diencephalon is connected to each lateral ventricle via a channel called the interventricular foramen.

Background:

  • Interhemispheric subdural hematoma (IHSDH) is an uncommon brain injury due to its unique location.
  • Despite rarity, IHSDH presents distinct clinical and radiological characteristics.

Purpose of the Study:

  • To investigate the clinical features, management, and outcomes of patients with interhemispheric subdural hematoma (IHSDH).

Main Methods:

  • Retrospective review of 42 patients with thick (≥3 mm) IHSDH diagnosed via CT or MRI between 2006 and 2015.
  • Analysis of clinical and radiological findings, treatment strategies, and patient outcomes.

Main Results:

  • The majority of IHSDH patients were male and over 60 years old, with falls being the most common cause.
  • Headache was the most frequent symptom; IHSDH often co-occurred with convexity subdural hematomas.
  • Favorable outcomes were observed in 27 patients, while six expired; conservative management was used in 22, and surgery in 10.

Conclusions:

  • Interhemispheric subdural hematoma (IHSDH) is a rare entity, particularly when isolated.
  • Patient outcomes are significantly influenced by the severity of the initial injury.
  • While surgery can address concurrent lesions, conservative treatment is generally favored for IHSDH.
