The vascular phase, also known as vasospasm, is the initial stage of hemostasis, crucial for preventing excessive bleeding when a blood vessel is injured. Background:

  • Spontaneous acute arterial subdural hematoma (SDH) is an uncommon condition.

Observation:

  • A 53-year-old male presented with sudden headache and dysphasia, followed by a seizure.
  • Imaging revealed an acute left-sided SDH with active contrast extravasation, indicating arterial bleeding.
  • No history of trauma was reported.

Findings:

  • Digital subtraction angiography confirmed active bleeding into the subdural space.
  • Surgical evacuation of the hematoma and coagulation of the bleeding perisylvian artery were performed.

Implications:

  • This case highlights the importance of considering spontaneous arterial SDH in the differential diagnosis of acute neurological deficits.
  • Early diagnosis and timely surgical management are critical for favorable outcomes in this rare neurosurgical emergency.
