Background:

  • Odontoid osteomyelitis is a rare infection affecting the C2 vertebra.
  • Cervical spine infections can present with nonspecific symptoms like headache and neck pain.
  • Venous thrombosis is a known complication of infections but rarely associated with odontoid osteomyelitis.

Observation:

  • A 71-year-old female presented with progressive left-sided headaches, neck pain, and nuchal rigidity over 3 weeks.
  • Neuroimaging revealed odontoid osteomyelitis with an associated epidural abscess.
  • Concurrent jugular vein and transverse sinus venous thrombosis was identified.

Findings:

  • The patient was diagnosed with odontoid osteomyelitis and epidural abscess.
  • Jugular vein and transverse sinus venous thrombosis were found to be secondary to the osteomyelitis.
  • This represents the first reported case of transverse sinus venous thrombosis secondary to odontoid osteomyelitis.

Implications:

  • Prompt diagnosis and treatment of odontoid osteomyelitis are crucial to prevent severe complications.
  • Recognition of venous thrombosis as a potential complication is important for timely management.
  • This case expands the understanding of infectious complications associated with cervical spine infections.
