Background:

Refractory frontal lobe epilepsy traditionally necessitates frontal lobectomy, a procedure associated with significant neurological deficits.
Developing less invasive surgical techniques is crucial for improving patient outcomes in epilepsy surgery.

Observation:

This study introduces a novel 4-step selective disconnection technique for refractory frontal lobe epilepsy.
The technique involves suprainsular window, anterior callosotomy, intrafrontal, and frontobasal disconnections, guided by electrophysiological mapping.
A case of a 9-year-old boy with right frontal lobe epilepsy due to focal cortical dysplasia is presented.

Findings:

The described periinsular anterior quadrantotomy (quadrantotomy) successfully treated refractory epilepsy in the presented case.
The patient achieved Engel Class I seizure outcome at 3-year follow-up.
Significant cognitive gains were observed post-operatively, alongside preserved motor function.

Implications:

Periinsular anterior quadrantotomy represents a potentially safer and effective surgical option for select cases of refractory frontal lobe epilepsy.
This disconnective technique may offer comparable seizure control to lobectomy with reduced morbidity.
Further research and application of this technique could advance epilepsy surgical management.
