- To highlight the implementation of MTM for older adults and underserved communities.
Main Methods:
- Student pharmacists engaged patients in MTM services over two academic years.
- Services included health screenings, personalized medication records (PMR), recommendations, action plans (PAP), and follow-up calls.
Main Results:
- 107 patients received MTM services, with a mean appointment duration of 62 minutes.
- Student pharmacists provided an average of 3.5 recommendations per patient.
- Follow-up revealed that an average of 2.6 recommendations per patient were implemented.
Conclusions:
- The KYM program successfully provided MTM to older adults and underserved populations.
- This initiative offers a model for other pharmacy programs to integrate MTM training and practical experience for students.
