Copeptin and insulin resistance: effect modification by age and 11 β-HSD2 activity in a population-based study

Age and 11β-hydroxysteroid-dehydrogenase-type 2 (11β-HSD2) activity influence the link between copeptin, a marker for arginine vasopressin (AVP), and insulin resistance. These factors, however, did not significantly affect the association with metabolic syndrome or diabetes.

- Arginine vasopressin (AVP) is implicated in metabolic syndrome (MetS) through effects on liver glycogenolysis, insulin/glucagon secretion, and ACTH release.
- AVP also upregulates 11β-hydroxysteroid-dehydrogenase-type 2 (11β-HSD2) in mineralocorticosteroid cells.
Purpose of the Study:
- To investigate if apparent 11β-HSD2 activity modulates the association between plasma copeptin (an AVP surrogate) and insulin resistance/MetS.
- To explore the role of copeptin in metabolic health within the general adult population.
Main Methods:
- A multicentric, family-based, cross-sectional study of 1089 adults (aged 18-90 years) in Switzerland.
- Mixed multivariable regression models analyzed associations of insulin resistance (HOMA-IR)/fasting glucose and MetS/Type 2 Diabetes with copeptin.
- Results were stratified by age and estimated 11β-HSD2 activity (urinary cortisol/cortisone ratio).
Main Results:
- Plasma copeptin levels were higher in men than women.
- HOMA-IR showed a positive association with copeptin when 11β-HSD2 activity or age was high, but not when they were low.
- Type 2 diabetes was positively associated with copeptin, but MetS was not.
Conclusions:
- Age and apparent 11β-HSD2 activity appear to modulate the relationship between copeptin and insulin resistance at a population level.
- These factors did not significantly influence the association between copeptin and MetS or diabetes.
- Further research is warranted to confirm these findings and elucidate underlying mechanisms.
