Area of Science:

  • Oral and Maxillofacial Surgery
  • Biomaterials Engineering
  • Regenerative Medicine

Background:

  • Temporomandibular disorders (TMD) involve the temporomandibular joint (TMJ), masticatory muscles, and associated tissues.
  • Therapeutic options for TMD range from non-invasive to invasive interventions.
  • This case report details the management of inflammatory joint pain and dysfunction via TMJ reconstruction.

Purpose of the Study:

  • To report a case of severe temporomandibular joint (TMJ) dysfunction treated with bilateral alloplastic prosthesis stock reconstruction.
  • To evaluate the efficacy of alloplastic TMJ replacement in restoring form and function.

Main Methods:

  • A 41-year-old female patient with a six-year history of bilateral TMJ pain and limited mouth opening underwent conservative treatment and arthrocentesis without improvement.
  • The patient then proceeded to bilateral discopexy.
  • Bilateral temporomandibular joint (TMJ) total reconstruction was performed using stock alloplastic prostheses following an established protocol.

Main Results:

  • The alloplastic TMJ replacement aimed to restore joint form and function, with pain reduction as a secondary outcome.
  • The procedure addressed symptoms including reduced maximum mouth opening and pain.
  • The patient's quality of life was improved post-operatively.

Conclusions:

  • Alloplastic TMJ replacement is a viable therapeutic alternative for severe TMJ conditions refractory to conservative management.
  • Restoration of form and function is the primary goal, with pain reduction being a significant benefit.
  • Further long-term studies are necessary to validate the sustained efficacy of total joint prostheses.
