Summary

Electrocorticographic high-gamma modulation (ECoG HGM) is specific but not sensitive for language localization compared to electrical stimulation mapping (ESM). However, ECoG HGM can reliably identify electrodes over language sites, with a pooled DOR of 6.44 and AUC of 0.77.

Background:

  • Presurgical language localization is critical for patients with epilepsy or brain tumors near language areas.
  • Electrical stimulation mapping (ESM) is the gold standard, but electrocorticography (ECoG) offers an alternative.
  • High-gamma modulation (HGM) analysis of ECoG signals is increasingly used for language mapping.

Purpose of the Study:

  • To compare the diagnostic validity of ECoG HGM with ESM for presurgical language localization.
  • To evaluate the sensitivity, specificity, and overall diagnostic accuracy of ECoG HGM.

Main Methods:

  • A meta-analysis of studies comparing ECoG HGM and ESM at the electrode level.
