Summary

Background:

  • Malignant cutaneous mixed tumors (CMTs) are rare adnexal neoplasms with biphasic differentiation.
  • Malignant transformation of benign CMTs (chondroid syringomas) is exceptionally uncommon.
  • Sebaceous differentiation has not been previously documented in cutaneous malignant mixed tumors.

Observation:

  • A case of malignant CMT with sebaceous differentiation on the scalp of an 81-year-old male is presented.
  • Histological examination revealed epithelial elements with ductal, cord-like, and gland-like structures, alongside atypical cells with pale/clear cytoplasm.
  • Focal sebaceous differentiation was noted, characterized by multivacuolated cytoplasm and nuclear indentations.

Findings:

  • The tumor exhibited a mixed stromal component with myxoid, chondroid, and calcified areas.
  • Immunohistochemical analysis showed strong and diffuse staining for cytokeratin 7 (CK7), epithelial membrane antigen (EMA), and androgen receptor (AR) in sebaceous cells.
  • Mismatch repair proteins (MSH6, MSH2, MLH1, PMS2) showed no loss of expression in the sebaceous cells.

Implications:

  • This case expands the histopathological spectrum of malignant cutaneous mixed tumors.
  • The presence of sebaceous differentiation in CMT offers new insights into adnexal tumor biology.
  • Further research may clarify the origin and behavior of CMTs with diverse differentiation patterns.
