PPIs Prevent Aspirin-Induced Gastrointestinal Bleeding Better than H2RAs. A Systematic Review and Meta-analysis

Proton-pump inhibitors (PPIs) are more effective than histamine-2 receptor antagonists (H2RAs) in preventing gastrointestinal bleeding and ulcers caused by low-dose aspirin (LDA). PPIs are the preferable choice when tolerated.

- A systematic literature search was conducted on PubMed, Embase, and Cochrane Central Register of Controlled Trials up to September 30, 2016.
- Included were randomized controlled trials and observational studies comparing H2RAs and PPIs for GI protection in LDA users.
- Data from nine studies on GI bleeding and eight on ulcer formation, involving 1,879 patients, were meta-analyzed.
Main Results:
- H2RAs were found to be less effective than PPIs in preventing LDA-related GI bleeding (OR=2.102).
- H2RAs also showed lower efficacy in preventing LDA-related ulcer formation compared to PPIs (OR=2.257).
- Statistical analysis indicated a significant difference in protective effects between the two drug classes.
Conclusions:
- PPIs demonstrate superior effectiveness in preventing both GI bleeding and ulcer formation associated with chronic LDA use.
- The findings suggest that PPIs should be preferentially prescribed over H2RAs for GI protection in patients taking LDA, provided they are tolerated.
