Antiasthma Drugs: Methylxanthines01:24

Antiasthma Drugs: Methylxanthines

1.7K
Theophylline, a member of the methylxanthine class of bronchodilators, has long been used in asthma management. While its exact mechanism of action is not fully understood, it is believed to have multiple effects on various cellular processes.
Theophylline is thought to inhibit phosphodiesterase enzymes, increasing intracellular levels of cyclic adenosine monophosphate (cAMP) and cyclic guanosine monophosphate (cGMP). This rise in cAMP and cGMP concentrations stimulates cardiac function,...

Dose-Response Relationship: Selectivity and Specificity

9.9K
Drugs exert their therapeutic effects by interacting with receptors, enzymes, or ion channels that are present throughout the human body. The strength and duration of the interaction between a drug and its target receptor are characterized by the selectivity and specificity of the drug. Selectivity refers to a drug's strong preference for its intended target over other targets. For instance, isoprenaline, a non-selective β-adrenergic agonist, interacts with both β1- and...

Adrenergic Agonists: Mixed-Action Agents01:28

Adrenergic Agonists: Mixed-Action Agents

1.6K
Mixed-action adrenergic agonists, like ephedrine and pseudoephedrine, directly and indirectly affect adrenergic receptors. These agents stimulate adrenoceptors and indirectly release stored neurotransmitters, amplifying the adrenergic response.
Ephedrine and pseudoephedrine lack a catecholamine group, making them less susceptible to degradation by metabolic enzymes. They have increased oral bioavailability and lipophilicity, resulting in a longer duration of action. Their response is reduced by...

Antiasthma Drugs: Leukotriene Modifiers01:19

Antiasthma Drugs: Leukotriene Modifiers

1.9K
Leukotriene modifiers, or cysteinyl leukotriene receptor antagonists, are medications used to manage chronic asthma. These agents target specific inflammatory mediators produced during arachidonic acid metabolism, an essential process in generating inflammation in the body.
Leukotriene modifiers work through two distinct mechanisms:

IV Infusion to Oral Dosing: Conversion Methods

The development of extended-release formulations has facilitated the transition from intravenous to oral medication, offering a more convenient and patient-friendly approach to drug administration. This transition, however, requires careful management to ensure that therapeutic drug levels are maintained, preserving efficacy and avoiding adverse effects. Understanding pharmacokinetic principles and dosage calculations is critical during this process.Pharmacokinetics of the...

Adrenergic Agonists: Direct-Acting Agents01:30

Adrenergic Agonists: Direct-Acting Agents

2.9K
Drugs that mimic the action of endogenous catecholamines like noradrenaline and adrenaline are called adrenergic agonists or sympathomimetics. Based on their mechanism of action, sympathomimetics can be classified as direct-, indirect-, or mixed-acting sympathomimetics. Direct-acting adrenergic agonists activate adrenoceptors without affecting presynaptic neurons, making them independent of neuronal catecholamine-depleting agents like reserpine and guanethidine.
These agents can be classified...

Area of Science:

  • Pharmacology
  • Respiratory Medicine
  • Drug Development

Background:

  • Theophylline is a common treatment for respiratory diseases but has a narrow therapeutic index and significant drug-drug interactions.
  • Doxofylline, a theophylline derivative with a dioxalane group at position 7, presents an alternative therapeutic option.
  • Elderly patients with comorbidities often face challenges with theophylline due to polypharmacy and potential interactions.

Purpose of the Study:

  • To compare the efficacy and tolerability of doxofylline with theophylline in treating respiratory diseases.
  • To elucidate the distinct pharmacological profile of doxofylline compared to theophylline.
  • To encourage further clinical trials investigating doxofylline as an alternative to existing therapies.

Main Methods:

  • Comparative efficacy and tolerability assessments.
  • Pharmacological profiling to identify distinct mechanisms of action.
  • Review of existing literature and clinical trial data.

Main Results:

  • Doxofylline demonstrates comparable efficacy to theophylline.
  • Doxofylline exhibits an improved tolerability profile and a favorable risk-to-benefit ratio.
  • Doxofylline shows a distinct pharmacological profile, lacking significant phosphodiesterase inhibition and adenosine receptor antagonism, and has minimal drug-drug interactions.

Conclusions:

  • Doxofylline is a viable alternative to theophylline for respiratory diseases, offering improved safety and tolerability.
  • Doxofylline's unique pharmacological properties differentiate it from theophylline, warranting its consideration as a distinct therapeutic agent.
  • Further randomized clinical trials are recommended to evaluate doxofylline's role in reducing exacerbations and hospitalizations in asthma and COPD, positioning it as an alternative to biologics and theophylline.
