    A rare fungal infection caused by Inonotus mold occurred in a neutropenic patient. This finding suggests Inonotus species should be recognized as a cause of invasive fungal disease in immunocompromised individuals.

    • Neutropenic patients with hematologic malignancies are susceptible to invasive fungal infections.
    • Basidiomycetes molds are an uncommon cause of invasive fungal disease in this population.

    Observation:

    • A case of lung-invasive fungal disease with possible cutaneous needle tract seeding is presented.
    • The causative agent was identified as the Basidiomycetes mold Inonotus spp.

    Findings:

    • Inonotus spp. can cause invasive fungal disease in neutropenic patients.
    • Cutaneous needle tract seeding is a potential route of infection for Inonotus spp.

    Implications:

    • Inonotus spp. should be considered in the differential diagnosis of invasive fungal disease in neutropenic patients.
    • This case expands the spectrum of known fungal pathogens causing severe infections in immunocompromised hosts.
    • Further research is warranted to understand the pathogenesis and treatment of Inonotus spp. infections.
