Background:

Picture Archiving and Communication Systems (PACS) are crucial for managing medical images in healthcare.
Evaluating PACS is essential to ensure technical, structural, and operational fitness.
New hospitals and developing countries often require specific PACS evaluation guidance.

Purpose of the Study:

To systematically review existing literature and propose a logical evaluation protocol for PACS.
To provide a tool for assessing PACS suitability, particularly for institutions in developing countries.

Main Methods:

Systematic review of 267 articles published between 2000 and 2017 from major scientific databases.
Data extraction and tabulation followed by review by three independent expert panels.
Statistical analysis of outcome data with reliability established at 0.82 using Pearson's correlation coefficient.

Main Results:

An optimal PACS evaluation protocol was developed, organized into nine key sections.
The protocol includes 20 essential components covering system configuration, network, storage, compression, image input, characteristics, presentation, communication, and security.
Expert panels ranked outcome data, ensuring a structured approach to PACS assessment.

Conclusions:

The developed protocol offers an objective tool for evaluating new or existing PACS in healthcare institutions.
It is particularly beneficial for healthcare settings in developing countries.
