This study presents a minimally invasive thymectomy technique using a uniportal subxiphoid approach with double sternal elevation for myasthenia gravis (MG). The method achieved extensive thymectomy with no mortality or morbidity in four patients.

Area of Science:

  • Thoracic Surgery
  • Minimally Invasive Procedures
  • Surgical Techniques

Background:

  • Myasthenia gravis (MG) is an autoimmune disorder affecting neuromuscular junctions.
  • Extended thymectomy is a key treatment for nonthymomatous MG.
  • Minimally invasive approaches aim to reduce surgical trauma.

Purpose of the Study:

  • To describe and evaluate a novel uniportal subxiphoid approach for extended thymectomy.
  • To demonstrate the feasibility of double sternal elevation for enhanced surgical access.
  • To assess the safety and efficacy of this technique in patients with myasthenia gravis.

Main Methods:

  • A 4-7 cm subxiphoid incision was used for dissection with videothoracoscopy.
  • Double sternal elevation was achieved using a sternal frame with two hooks.

  • Pericardial and mediastinal structures were carefully dissected to facilitate superior hook insertion.
  • Main Results:

    • The technique was applied to four patients between January and April 2017.
    • Complete removal of anterior mediastinal fatty tissue and the aorta-pulmonary window was achieved.
    • No mortality or morbidity was observed in the patient cohort.

    Conclusions:

    • The uniportal subxiphoid approach with double sternal elevation allows for extensive thymectomy.
    • This technique offers a safe and effective minimally invasive option for myasthenia gravis treatment.
    • Further studies are warranted to validate these findings in a larger patient population.
